Health and Wellness Magazine • 57 What if my ultrasound shows uterine fibroids? As many as three out of every four women will develop uterine fibroids during their childbearing years. Most fibroids, or leio- myoma, regress after menopause and do not need treatment. Indications for treatment can include pain, constipation, urinary symptoms, abnormal bleeding, rapid change in size, or inability to evaluate the ovaries by exam or ultrasound. First line treatments for fibroids are contraceptive pills and hormone-containing intrauterine devices. The minimally- invasive surgical option of endometrial ablation (destruction) of the uterine lining can be considered if the fibroids are small. Uterine artery embolization, or blocking of the uterine vessels, has very strict criteria for its application, but in the correct patient population can decrease fibroid size by about two- thirds. Surgical removal of just the f i b r o i d s ( m y o m e c t o m y ) a n d surgical removal of the uterus (hysterectomy) are other options. If a surgical option is chosen, a minimally-invasive approach performed by an experienced laparoscopist is best, according to the American Association of Gynecologic Laparoscopists. Many laparoscopic surgeries can be performed with the assistance of a robot, which has been shown clinically to decrease length of hospital stay, decrease blood loss at the time of surgery, and decrease the use of pain medications after surgery. Most patients require a few-hour or overnight stay after laparoscopic robotic surgery, and many patients can return to work after a couple of weeks of recovery.
